If you’re an SSS (Social Security System) female member, you can avail their maternity benefits if you qualify to their qualifying conditions prior to submission of application and requirements.
For employee and employer, once a member gets pregnant she should inform company HR (usual department to assist you in this kind of matter) then accomplish SSS Maternity Notification Form (MAT-1) and certificate of proof of pregnancy.  Your employer will notify SSS via notification forms you submitted.

For self employed, un-employed and voluntary members they can go directly to their nearest SSS branch to comply above required documents. In case of failure and delay of notification, maternity benefit application may be denied.

SSS Maternity Benefit Application Required Documents


For Notifications
  1. Maternity Notification Form (MAT-1)
  2. 2 valid IDs both with signature and at least one 1 with photo and date of birth
  3. Proof of pregnancy / Ultrasound reports / Certification of Pregnancy from your doctor
For Claim and Reimbursement

  1. Maternity Notification Form duly stamped and received by SSS
  2. Maternity Reimbursement Form (MAT-2)
  3. 2 valid IDs both with signature and at least one 1 with photo and date of birth
  4. Child Birth Certificate original copy from NSO / Local Civil Registrar with the original "Certified True Copy" stamp or mark

Additional Requirements required by SSS


  1. If normal delivery or CS - certified true or authenticated copy of duly registered birth certificate. In case the child dies or is a stillborn, duly registered death or fetal death certificate.
  2. If Delivery was through Caesarean - certified or authenticated copy of duly registered birth certificate and certified true copy of operating room record/surgical memorandum.
  3. If Miscarriage or abortion - obstetrical history stating the number of pregnancy certified by the attending physician and dilatation and curettage (D&C) report for incomplete abortion, pregnancy test before and after abortion with age of gestation and hystopath report for complete abortion.

Gandang tanghali po..ask ko lang po 7months pregnant napo ako ngayon..makakapagfile papo ba ako ng sss maternity benifits un employeed napo ako ngayon?tapos kong sakali po ba na maapprove back to zero po ba ang contributions???

Ask lang po Ako,nakapag fill up na po Ako sa maternity ko at napa sa ko na po Yong requirements.nalaman ko din if ilan Ang maternity ko .Yong pag claim po ,ano ba yon ,ipasok ba yon sa account ko Yung pera?
