Same goes with other individual employed or non-employed who wants to apply and be part of Social Security System. The first thing you’ll need to do prior to getting that is by filling up that E-1 form with your personal details including the beneficiaries, signature and thumb marks. Do note that before proceeding to any SSS branch you’ll need to prepare the documents needed to process your application.

SSS E1 Form Requirements to get SS Number
Original
or certified true copy and photocopy of the following supporting documents:
- NSO Birth Certificate
- Baptismal Certificate (in absence of birth certificate)
- Valid Passport
Other
required secondary documents if you don’t have any of the above (2 copies
needed):
- Record of Employment/Employer ID
- GSIS Member’s Record
- Certification from National Archives
- Alien Certificate of Registration
- Marriage Contract
- Birth Certificate of children
- School/Voter’s Identification Card
- Driver’s License
- Joint Affidavit

filling up E1 form, make sure to double check the spelling of your personal details
to avoid future problems when you avail SSS services and benefits. After
submission of form, requirements and details verification, they will give you
the SSS number filled or written on the upper left of the E1 form. I believe
they will give you the duplicate or pink copy for your record keeping. Please
secure and avoid this copy from getting lost as this will be your reference in
case you forgot your SS number.
