If you’re already hired on your first job, you are required to get SSS number and for you become a member you need to fill up an E1 form to get that SS number. The same goes with other individuals employed or non-employed who want to apply and be part of the Social Security System. The first thing you’ll need to do before getting that is fill up that E-1 form with your personal details including the beneficiaries, signature, and thumb marks. Do note that before proceeding to any SSS branch you’ll need to prepare the documents needed to process your application.
SSS E1 Form Requirements to Get SS Number
Original or certified true copy and photocopy of the following supporting documents:
- NSO Birth Certificate
- Baptismal Certificate (in the absence of birth certificate)
- Valid Passport
- Seaman's Book (Seafarer's Identification and Record Book)
- Professional Regulation Commission (PRC) card
Other required secondary documents if you don’t have any of the above (2 copies needed). Both documents must have the correct name and at least one (1) with date of birth.
- Record of Employment/Employer ID
- ATM card (with cardholder's name)
- Bank Account Passbook
- GSIS Member’s Record
- Certification from National Archives
- Alien Certificate of Registration
- Marriage Contract
- Birth Certificate of children
- School/Voter’s Identification Card

- Government Service Insurance System (GSIS) card/Member's Record/Certificate of Membership

How to Get SSS E1 Form
You can visit your nearest SSS branch or download it online via the SSS official website E1 Form file. It is a PDF file, you need a PC or smartphone with a PDF editor to fill it up. You can also print the file and then fill it up using a ballpen.
Instructions:
- Fill out the E1 form and submit to the nearest SSS branch office together with the required documents.
- Fill out the applicable portions as follows: Parts I-A, B and D, if applying for SS number as pre-employment requirement and Parts I-A, B, C, and D, if applying for Self-Employed, Overseas Filipino Worker (OFW) or Non-Working Spouse membership.
- Always indicate "N/A" or "Not Applicable", if the required data is not applicable.

When done filling up the E1 form, make sure to double-check the spelling of your personal details to avoid future problems when you avail of SSS services and benefits. After submission of the form, requirements, and details verification, they will give you the SSS number filled or written on the upper left of the E1 form. I believe they will give you a duplicate or pink copy for your records. Please secure and avoid this copy from getting lost as this will be your reference in case you forgot your SS number.
Note:
- New registrant who is over sixty (60) years old and not a surviving spouse pensioner/guardian of a pensioner, is not qualified to apply for an SS number.
- Your SS number is your lifetime number. You should not have more than one SS number.
- The following required documents should be the original or certified true copy issued by the City or Municipal Civil Registrar or Philippine Statistics Authority/National Statistics Office:
- Birth Certificate
- Marriage Contract/Marriage Certificate
- Death Certificate
- All identification (ID) cards and/or documents with English translation issued by foreign governments are acceptable.

Once you have your SS number, you need to register and create an account on the SSS official website. This will help you get updated if your monthly contributions are reflected on SSS or not. It will also help you monitor your employer’s contributions to your account.
